***NOTE: Grade Change Forms are not to be given to students under any circumstances.
To access the Grade Change form, click on the link below or enter the URL in your web browser and press the “Enter” key on your keyboard:
Grade Change Form
A window will appear on your computer screen and you will be prompted to enter your “User name” and “Password.”
Enter “fscad\” followed by the Windows user name and password you use to log on to your computer. (Example: User name: fscad\bscholten; Password: xxxxxxx)
(You have three opportunities to successfully access the grade change form. If you enter the wrong user name and/or password three times in succession, access will be denied and you will have to try again another time.)
The Grade Change form will appear in PDF format. Simply click on the printer icon near the top left corner of the screen to print out a copy of the form.
The college uses the numerical grading scale below; letter grades are not used.
Undergraduate grades are (effective Fall, 2007):

1.5 - Below average but passing

0.0 - Failing
Graduate grades are:
4.0 - Superior
3.5 - Good
3.0 - Good
2.5 - Passing
2.0 - Passing
0.0 – Failure
Please fill out the form completely, including your signature and that of your dean, department chair or program chair, and return it to the Registrar’s Office immediately for processing. Grade change forms that are not filled out completely will be returned.
Please remember to log completely out of your Internet connection (close the session) by clicking on the red “X” button located in the extreme top right corner of the screen.
