PayInfo Update

PayInfo is a web-based tool for Commonwealth employees (state benefited and contractors) to access payroll information through the internet.  PayInfo is accessible using a PC with and internet connection.  It is available 24 hours a day, 7 days a week.  Payroll information is available on the Thursday before the Friday payday.  PayInfo now includes 30 pay periods of data and an unofficial online copy of the W2 form.  For additional information, please see the attached brochure PayInfo Brochure 2006.    You may connect to the PayInfo system at the following link Fitchburg State College - Payroll - PayInfo.


One Card

Fitchburg State College invites all part-time and temporary employees to participate in the "One Card" system. This card is used for library privileges, access to the Recreation Center and other areas of interest, as well as for identification purposes.

An employee must be registered in the campus-wide Banner System to be eligible for the "One Card". We, therefore, ask that you complete a "Authorization for Employment" form with the required information. Please forward this completed form to the Payroll Office.

All new employees will be required to complete the "Authorization for Employment Form," Commonwealth of Massachusetts Standard Contract, Commonwealth of Massachusetts Terms and Conditions, Health Insurance Responsibility Disclosure Form, Direct Deposit Forms, I-9 Form, State and Federal Tax Forms and provide a copy of your Social Security Card. For your convenience, all necessary forms can be found on our website at Download from Form Section.

State Board of Retirement Contract Service Buy Back

(M.G.L. c.32, §4(1)(s))

To purchase creditable service for service provided to the Commonwealth as a "contract employee", a member of the State Employees' Retirement System ("SERS") must meet the following requirements:

·   Must currently be a member in service of SERS with at least ten (10) years of state service. (This does not include other service time with cities, towns or counties of the Commonwealth).

·   The contract employee service being purchased must have immediately preceded membership or re-entry into SERS.

·   The contract service being purchased must have been service to the Commonwealth, not service to a city, town, county or other governmental entity.

·   The job description of the contract service position must have been substantially similar to the job description the member held upon becoming an employee and a member of the SERS.

·   The maximum amount of contract service eligible to be purchased is four (4) years.

    Eligible part-time contract service shall be credited on a proportional basis.

·   The cost to purchase contract service is the amount equal to that which would have been withheld as regular retirement deductions for the contract service as if the service had been rendered as a state employee, and the individual had been a member of the SERS, plus buyback interest of 4.125%.

Prior to purchasing any eligible contract service, a member must have the attached form State Retirement Contractor Buy Back Form 5 completed by the State agency where the contract service was provided and submit the required documentation to the State Retirement Board for review and approval.

No creditable service shall be allowed unless the member provides documentation of the contract service satisfactory to the State Retirement Board's requirements.

Once the member is notified by the Board that he/she is eligible to purchase contract service, he/she must within 180 days after the notice either: (1) purchase the service via a lump sum payment; or, (2) enter into an installment payment agreement.

Please note, if the 180-day period expires without action by the member, the contract service will no longer be eligible for purchase.

HIRD (Health Insurance Responsibility Disclosure Form)

Effective 7/1/2007, the Commonwealth of Massachusetts requires all adults who can afford health insurance to have coverage. All part-time, non-benefited contractors paid through the State Payroll System (HRCMS) should visit the Commonwealth Health Connector's website at www.mahealthconnector.org or call 1-877-MA-ENROLL to compare health insurance options and are eligible to purchase coverage on a pre-tax basis through a Section 125 plan. The Commonwealth of Massachusetts' Employer ID# is 149683. All employees that do not enroll in Connector Authority health coverage must complete the the Commonwealth of Massachusetts HIRD form.
