Distributed Learning Request Forms
For use by Fitchburg State College faculty and students enrolled through Distributed Learning, either Distance Learning or Extended Campus.

Faculty members use this form to schedule their classes for a session with a librarian. This request form is for on campus classes only. Learn more about our on campus Instruction program.
Library Instruction Services for Distance Learning and Extended Campus: Through our Distributed Learning Library Services off-Campus Libray Orientation and Instruction is available for any of your courses taught through Distance Learning or at Extended Campus sites. Request Use of Library Instruction Classroom
Use this form to reserve the Library Instruction Classroom for your own class or meeting; priority will be given to Library sessions and academic classes.
Send Reference & Research Questions
Use this form to send questions to our reference staff via email. A librarian will respond as soon as possible, during Reference Desk hours of operation.

Display Space Reservation Request Form
Use this form to request permission. The approval for displays will be at the discretion of the Library Director. Displays proposed by campus-affiliated groups or individuals will receive preference over external groups. See the complete Display Space Policy for more information.
Filming/Photography Permission Request Form
