Health-immunization
Requirement
Massachusetts Public Law 105 CMR 220.600 requires the
following immunizations:
Full-time matriculated undergraduate and graduate students; part-time undergraduate and graduate students in health science programs who may be in contact with patients; and full-time or part-time students on a visa.
- Completed Fitchburg State College
Health Form including a physical exam that has been completed
within the past year;
- Two (2) doses of live measles vaccine
(one after 12 months of age and update) or positive titer
- At least one dose of mumps and rubella
vaccine or positive titer
- TD (tetanus & diphtheria booster)
within the past 10 years
- Tuberculosis risk assessment/PPD
if required. College policy states that PPD must be done in the USA. If PPD (TB Skin Test) is positive, we need proof
of a negative chest X-ray performed in the USA.
- Three doses of hepatitis B vaccine or positive
titer
- Meningitis vaccine or waiver required for residential students (newly enrolled full-time students who will be living in a dormitory or comparable congregate living arrangement.)
Part-time and Full-time Matriculated Undergraduate and Graduate
Nursing Students.
- Annual physical
- Annual PPD (TB test) and proof of
a negative chest x-ray if the PPD test was positive.
- Need the state required immunizations as stated above #2-6.
- Copy of a positive hepatitis antibody titer.
Failure to comply with these requirements
will prohibit you from registering for classes.
